Coffee Talk: Health System Transformation

On June 6, 2019, the Connecting Care Act, 2019 came into force. In the first wave of Ontario Health Team Self-Assessments, over 150 potential Teams made submissions. Since that time, 24 have been advanced through the Application process and into Implementation. 41 applicants were designated as “In Development”. Those In Development have been asked to complete and submit Progress Reports to the Ministry in January 2020.

To support providers In Development to advance their Ontario Health Teams to Full Application, we will be hosting a Coffee Talk – Health System Transformation webinar on Tuesday, January 14, 2020 at 12:00 p.m. entitled “In Development Progress Reports: Tips for Advancing Your Ontario Health Team”, presented by Karima Kanani.

The session will cover:

  • Update on Ontario Health Teams 
  • Review of Key Features of OHT Self-Assessment and Application 
  • Tips for Advancing Your OHT in the Development Process

This program is eligible for up to 1.0 Substantive Hour of Continuing Professional Development (CPD) credits with the Law Society of Ontario (LSO). Note: Participants intending to claim LSO CPD credits are required to register individually and participate in the live webinar.
